Registered Nurse (RN) – Home Care Case Manager

About the Role:
Lead personalized care plans for patients with chronic or post-operative needs, supervising caregivers and ensuring clinical excellence.

Responsibilities:

• Develop and update individualized care plans.
• Administer medications, wound care, and IV therapy.
• Supervise and mentor caregivers/CNAs in the field.
• Educate families on home care protocols.
• Conduct client assessments and adjust care as needed.

Requirements:

• Certification: Valid TTNMC registration.
• Experience: 2+ years in home health or geriatrics.
• Traits: Strong leadership, clinical judgment, and empathy.
• Availability: Weekdays with on-call flexibility.
